Drug Disposal Survey

The Joint Commission on Health Care is requesting your participation in a survey.
In 2018, the Joint Commission on Health Care (JCHC) was asked to study whether to require
many retail/community pharmacies to accept for disposal prescription drugs that have been
previously dispensed. The retail/community pharmacies that the JCHC was asked to focus on are
those that:

  •  dispense Schedule II or III controlled substances;
  •  do not primarily dispense by mail, common carrier, or delivery service; and
  •  are not located within a hospital.

The goal of this survey is to understand your perspective on requiring many retail/community
pharmacies to accept for disposal unused prescription drugs.
Your participation in this study is voluntary. By completing the questionnaire you are
voluntarily agreeing to participate. You are free to decline to answer any particular question you
do not wish to answer for any reason.

The survey is anonymous and confidential. We are not collecting any individually identifiable
information. No one will be able to identify you or your answers, and no one will know whether
or not you participated in the study. Your survey responses will be combined with those from
other participants and analyzed as a group.

The survey takes about 5-15 minutes to complete.
